Our Caring Staff
Brenda supervises all departments and oversees all operations at Chestelm Health & Rehabilitation Center. She is ultimately responsible for the welfare of each and every resident here.
Family members, residents, or responsible persons are encouraged to call if they should have any questions or concerns relating to any part of the functioning of the facility or any of its employees.
Deanna can assist you with the admission process, including particular room accommodations and readmissions following a hospital stay. Also, Deanna will be happy to assist you with financial concerns, including the benefits of medicare coverage and other insurance.
Deanna welcomes you to Chestelm Health & Rehabilitation Center and will assist you with getting settled, personal belongings, and any admitting concerns.
Officer Mark Epright, JD, LLM
Mark can assist you with any questions you may have regarding the financial decisions of the facility. Mark also serves as a Compliance Officer. Any "red flag" issues should be brought to his attention immediately.
Director of Nursing
Doreen Flaherty, RN
Doreen is responsible for the overall operations of the nursing department, including the activities of the registered nurses, licensed practical nurses, and certified nursing assistants.
Doreen monitors the direct medical care rendered and is available to answer any of your medical questions, including a review of your loved one's medical records upon request.
Doreen also serves as a Quality Assurance Director, providing ongoing assessment of current facility policies and procedures. She is a "hands-on" leader here.
Sylwia Korczak, Occupational Therapy Assistant
Sylwia oversees the daily operations and treatments provided by the Rehab Department, including coordination of occupational, physical, and speech therapy services. Sylwia works closely with the nursing staff to monitor the status of all residents at Chestelm Health & Rehabilitation Center.
Sylwia ensures that a rehab assessment is completed quarterly on all residents to ensure their highest level of functioning.
Michael Percy LPN
Michael assists the Director of Nursing in the daily management of patient care. He rounds with our Staff Physicians, keeping patients, families, and staff informed of the current health status. He also coordinates the education of our employees and ensures their educational development. He coordinates our resident care conferences where the current health statuses are exchanged with residents / patients, families, community caregivers, discharge planners, and our leadership team.
Jodie Veilleux, RN
Jodie works closely with our Director of Nurses in the completion of our MDS’s. The MDS is a powerful tool for implementing standardized assessments and for facilitating care management in nursing homes. The MDS is used for many quality initiatives.
Office Manager / Accounts Receivable Coordinator
Mary is responsible for the day-to-day financial and business operations of the office. Mary can assist you with any financial considerations, including Medicare, Medicaid, and private billing. She can also direct you accordingly in regard to patient funds.
Therapeutic Recreation Director & Volunteer Coordinator
Marie supervises an astonishing variety and volume of recreational and social activities at Chestelm Health & Rehabilitation Center.
Additionally, she oversees the following programs: Alzheimer's Support Group (Dementia Consultant), palliative care and holistic care coordinator, reiki master, and music therapy volunteer coordinator.
Deanna can offer you guidance and support with adjustment to the facility, roommate concerns, and psychosocial well-being. Deanna can also offer you assistance with discharge planning needs to include an arrangement of home care and adult day care as well as other needed community support.
Food Service Supervisor
Kathy oversees the Dietary Department. She is a certified Food Service Director. She continually meets with all of our residents regarding their food concerns or preferences. You can contact Kathy if you are interested in planning a special lunch or family party.
Rebecca Iselin, RD
Rebecca visits all residents upon admission to assure the resident is on the most appropriate diet. Every effort is made to accommodate food preference and maintain the residents' usual eating pattern. All residents are constantly reviewed to monitor their overall nutritional status.
The Environmental Coordinator oversees the Maintenance Department, Housekeeping Department, and Laundry Department of Chestelm Health & Rehabilitation Center. Bruce is responsible for fire safety and all safety issues pertaining to our residents, staff, and visitors. Contact him with any concerns.
Adult Day Care Services
Liz is responsible for the day-to-day operation of Chestelm Adult Day Services. She handles all registrations of new clients, coordinates their care with their attending physicians, and sees that all of their personal and medical needs are met while attending the Day Care Center.
