Effective Long-Term Rehabilitative Care

You can get your treatment for medically complex issues from our skilled team of caregivers. We will work with you to treat your multiple medical issues and resulting impairments. All your needs will be addressed through our large range of specialized clinical rehabilitation programs.
 
State-of-the-art equipment will be used as forms of treatment, including ultrasound, diathermy, and electrical stimulation. The team at Chestelm Health & Rehabilitation Center
and Preferred Therapy Solutions will take an interdisciplinary approach to helping you in stroke recovery through a holistic recovery plan.
 
Our interdisciplinary team of caregivers has experience in bariatric management to assist you in becoming more independent and functional. You'll also receive help with conditions such as COPD, emphysema, and post-open heart surgery through our cardiac and pulmonary recovery plan.

Specializing in Long-Term Post-Acute Care

When you are discharged from the hospital, you'll need to be closely monitored and go through rehabilitation to ensure you safely return home. You'll need to receive post-acute care.
 
Some post-acute services include post-operative, acute episodes of chronic conditions, medical management, safety management, and strength and conditioning.

You are in good hands at our facility. Our facility is staffed 24 hours by registered nurses (RNs), licensed practical nurses (LPNs), and certified nursing assistants (CNAs).
 
Each employee's average length of stay is over 5 years, so you'll receive excellent care from our credentialed staff who go through extensive background checks.

Excellent Dietary Recommendations

Our Resident Food Council members and our Food Service Director design our menus according to the preferences of the residents. Our menus are nutritionally balanced by our dietitian.
 
Specialized diets are easily accommodated. Our residents are offered choices daily through our a la carte dining service. Patients / residents are more than welcome to request items or recommend recipes.
 
It is our philosophy to keep diets as liberal as possible. Nutrition is a key component to healing. We use fresh produce and have an “always available menu” to choose from.

Discharge Planning Specialists

Comprehensive discharge planning begins the day you are admitted. Our team of specialists will evaluate and develop a unique plan of care. Our team will work in conjunction with you and your family, friends, or community resources to provide a safe and successful discharge.

Quality Assurance Performance Involvement (QAPI)

It is our policy to develop, implement, and maintain an ongoing program designed to monitor and evaluate the quality of care provided, to develop and pursue methods to improve quality care, and to resolve identified problems.
 
There are numerous internal tools that we use which help us focus on specific areas. Two tools which our residents and patients have access to are our satisfaction surveys, which are mailed quarterly and upon discharge, and our suggestion form, which is located in the hallway by the rehabilitation department.
